Homelessness Prevention Application

If you are a household with children and about to become homeless, Path Home’s Prevention Program may be able to help you stay in your home by providing assistance with current or back rent. This program is available to families who live in the Portland, Oregon metro area.

Once you have completed the application, submit it by email, fax, mail, or in person:
